Function

Lamivudine can metabolize into lamivudine triphosphate in HBV infected cells and normal cells. It is the active form of lamivudine, which is both the inhibitor of HBV polymerase and the substrate of this polymerase. Lamivudine triphosphate is incorporated into the viral DNA chain, blocking the synthesis of viral DNA, and does not interfere with the metabolism of deoxynucleoside in normal cells α and β Its inhibitory effect is weak, and it has little effect on the DNA content of the mammalian chemicalbook cells. It has no obvious toxicity to the structure, DNA content and function of mitochondria. The detection results of serum HBV DNA in most patients with hepatitis B showed that lamivudine could rapidly inhibit HBV replication, and its inhibition continued throughout the treatment process. At the same time, the serum transaminase is reduced to normal. Long-term application can significantly improve the inflammatory changes of liver necrosis and alleviate or prevent the progress of liver fibrosis.

 

Application

Lamivudine, as an antiviral drug, is used for the treatment of hepatobiliary diseases caused by hepatitis B virus infection. Randomized controlled clinical trials at home and abroad showed that oral administration of 100 mg per day could significantly inhibit the level of HBV DNA, and the HBeAg seroconversion rate increased with the prolongation of treatment time. The HBeAg seroconversion rate was 22%, 29%, 40%, 47% and 50% for HbeAg positive patients after 1, 2, 3, 4 and 5 years of treatment, respectively; The serum conversion rate of HBeAg is also higher in patients with higher ALT level before treatment. Long-term treatment can reduce inflammation and reduce the incidence of liver fibrosis and cirrhosis. Randomized controlled clinical trials showed that this drug can reduce the incidence of liver function decompensation and HCC (hepatocellular carcinoma). In decompensated cirrhosis patients can also improve liver function and prolong survival. Foreign research results show that lamivudine has similar efficacy and good safety in treating chronic hepatitis B in children and adults.
For patients with hepatitis B liver transplantation, lamivudine was used before transplantation; After transplantation, lamivudine combined with HBIG can significantly reduce HBV reinfection after liver transplantation and reduce the dose of HBIG.
